I hereby authorize Affinity Cellular to store and charge the Account Holder and/or Cardholder's Payment Account information, monthly, with no specified end date, or until an approved disconnection request is submitted via phone or email by the customer to an approved Affinity Cellular customer support representative.
I hereby authorize Affinity Cellular to charge the Account Holder and/or Cardholder's Payment Account information for variable recurring monthly payments for cellular service and/or equipment or services provided, including but not limited to, Return Merchandise Authorization charges for non-compliance returns, purchased Affinity Cellular provided equipment that has been damaged or is in any form different from what was provided upon delivery, past-due account balances for monthly recurring service charges and/or purchased equipment, and/or shipment fees associated with customer purchases and/or returns.
